Choose any action, up to five seconds, and declare a circumstance under which you will take that action. The length of this action is the same as the action you ready, plus one second for the Ready. When the chosen circumstance becomes true, you may take the declared action as a Free Reaction.
If your next turn starts, and the circumstance has not become true, you lose the readied action.
Sir Roland examines the battlefield, and comes to the conclusion that Aurelia is likely to be charged by one of the goblins that they are facing. He moves himself to stand between the goblins and Aurelia, and declares a Readied Action: “If a goblin moves into a square that I can reach, I will attack him.”
One goblin attempts to rush Aurelia and moves into a square that Sir Roland can reach. Sir Roland's Readied Action triggers, and he swings at the goblin. The goblin (somehow) survives, and continues trying to move past Sir Roland, thus provoking a Counterattack as he leaves the area that Sir Roland threatens.
