You clasp hands with another creature, and take their oath.  This oath is usually to avoid doing a specific thing, but can be any oath that both you and the target creature agree upon.

If the target creature ever violates the terms of their oath, they immediately take 3d6 Vital damage directly to hit points, and the spell ends.

Note that the target of this spell must be willing to take the oath, but this can include a creature that is coerced.  For example, a foe who, after being defeated, agrees to swear never to harm the Spellcaster again could be bound by this spell.  The target of the spell will be aware of the consequences of violating the oath.
